If an individual refuses to complete the form with required information, their service will end on the expiration day defined for all service members.

The text of the oath contains fillable fields that a filer must complete with corresponding dates, service unit they’d like to serve at. Additionally, the form is signed by the warrant officer or the member of the Army National Guard.
